Hospital discharge

A ride home, or to a receiving facility, once the rider is cleared to leave.

  • Requested by unit staff, case management or a family member
  • Tell Dispatch the entrance where the driver should report
  • Drop-off at home, a skilled nursing facility or assisted living
  • Tell Dispatch who to call on the unit if discharge timing changes

Facility-to-facility transfers

Moving a patient or resident between two care settings.

  • Sending unit and room, receiving unit and contact
  • Ambulatory or wheelchair transport, or stretcher confirmed by Dispatch
  • Timing coordinated with both sides by phone

Recurring transportation

Repeating trips on a set schedule — dialysis, infusion, wound care, therapy.

  • One schedule set up with Dispatch instead of a request per ride
  • Return trips arranged around the treatment center’s timing
  • Schedule changes handled by phone

Send the mobility level with the request. It determines the vehicle assigned.

Ambulatory

For riders who can safely transfer into a standard vehicle seat, with boarding assistance as needed. Walkers and canes travel with the rider.

Wheelchair

Ramp- or lift-equipped transportation with securement for riders who remain in their wheelchair during transportation. Tell Dispatch whether the chair is manual or power.

Stretcher

For riders who must remain lying down during transportation. Stretcher trips are not part of the online form.

Before you call

What Dispatch Needs

Have these details ready when you contact Dispatch.

  1. Rider name

    The name the receiving facility and the payer have on record, plus a phone number for the rider or caregiver.

  2. Mobility level and equipment

    Whether the rider walks with assistance, stays in a wheelchair, or must remain lying down. Include chair type, walker, cane or oxygen.

  3. Pickup facility, unit and room

    Facility name and address, unit and room number, and the entrance or department where the driver should report.

  4. Destination and receiving contact

    The drop-off address, and for a transfer the receiving unit and a contact name at the receiving facility.

  5. Appointment or discharge timing

    The appointment time, or the time the rider is expected to be ready. Note whether a return trip is needed.

  6. Payer details

    Health plan and any authorization number, the facility name and billing contact, or a note that the trip is private pay.

  7. A contact for changes

    A name and a direct number at your facility that Dispatch can reach about timing changes.

Tell Dispatch which applies when you send the trip.

Facility billing

Facility billing may be available for trips arranged by healthcare organizations. Choose facility billing on the request form and enter the facility name and a billing contact. No personal card is collected.

Health-plan authorized trips

Send the plan name and any authorization or reference number with the request. Coverage and authorization are decided by the plan.

Confirm details with Dispatch

Private pay

The trip is quoted before it is scheduled. A card is saved with the request. Nothing is charged when the request is submitted. The charge is made after Dispatch approves the trip.
